At Paso Petcare, we pride ourselves in providing excellent service from the moment we answer your call. We do operate by scheduled appointment, which allows us to minimize waiting times and maximize your time with the doctor during each visit. Our Hospital Policy, Client Information, and Patient Information forms are available online so you can complete them at your convenience.

Review and print the Hospital Policy form. Open the Client Information form and fill in all highlighted spaces. If your mailing address is different than your physical address, please provide both. An Authorized 3rd Party is someone 18 years-of-age or older that you designate to act on your behalf to bring your pet in for services or pick up prescriptions, etc. and they MUST be listed on your account for us to be able to speak to them. Open the Patient Information form and fill in all highlighted areas. Print the completed forms and sign / initial where indicated.

Once you have an appointment scheduled, email the documents to info@pasopetcare.com or fax them to 805-238-1971 – please include your appointment date. In addition, any other medical history from your prior vet is helpful, so please ask them to send records in advance of your appointment or simply ask us to request the records for you and provide their contact information.

PLEASE NOTE: It is required that the legal owner or guardian (at least 18 years old) be present for the first appointment.
