Once you have an appointment scheduled, please email the completed/signed documents to info@pasopetcare.com or fax them to 805-238-1971 – please include your appointment date. In addition, any other medical history from your prior vet is helpful, so please ask them to send records in advance of your appointment or simply ask us to request the records for you and provide their contact information.
